Visitor Sign-in and Out: All visitors will sign in with their name and contact information upon arrival.įacemask Required: Residents will wear a mask as tolerated, and visitors will always wear a mask over both their nose and mouth. If you have been around an individual who has confirmed COVID-19, is waiting for results of a COVID-19 test, or if anyone in your house is ill with the above symptoms, you may not enter.Įntry Screening: Visitors entering the building will self-screen for signs and symptoms, known exposure to COVID-19, and a temperature reading before proceeding through the building. We ask you to enter only the areas of the building required for your visit.

Guests should stay at least six feet from residents, except the one with whom they are visiting.

We encourage guests to visit within the resident room with the door closed.

All visitors need to sign in, prescreen, wear source-control masks, and perform proper hand hygiene when entering the building.

We must continue to limit the risk of disease transmission and keep all residents and employees safe.
